How do I get the hospital chat tab on the bottom of my screen to talk to people?

You need to be in the Hospital to have that chat active. Hardly any conversation happens there though, as people usually don't last long in there.

You need to be in the Hospital to have that chat active.

Click the Blue Settings Icon in the bottom right hand corner by your chat windows. > "Jail & Hospital" then click Enable and whether or not you want it enabled with or without the notifier, is up to you. I'd agree with Hoy though, not much action there so most people leave it disabled.
