This happens due to "Distraction" effect. If you had more than one attacker within 30 seconds, the distraction effect may cause you to skip some hits. There was an attack on you prior to this log, within 30 seconds of the double hit, which resulted in Distraction effect being applied to you.
If I'm reading this right. If I attack someone, lose, and re-attack, my 1st hit will have level 1 distraction?

No, because it is the same person doing the hit. Distraction effect only kicks in when there are two or more players involved in attacking a single person. So if you attack me, take an action (hit/miss/temp) and then lose, if this is followed up by another player attacking me within 30 seconds of you taking an action, I will have distraction effect active.
