I always see Duke at level 5 now. And I don't know when people group attack him.

When I had that missions, the way to do it was the wait until Duke is a few seconds away from reaching Loot Level 4, and then join the fight. When you see other people are starting to join, join as well, throw a smoke grenade, and then wait until Duke gets beaten by the other people in the fight. If you're feeling optimistic, you can try and hit him with a high accuracy weapon like the macana.
However, that doesnt seem to be the case now because he's at loot level 5 and not in hospital. Maybe the mechanics have changed.

The good loots from NPCs are really good. But rare.
Also with the recent change in gym training less players will be attacking.
More players are training and maybe a lot of looters are seeing they dont have much chance of looting something worth big money.
The npcs will still be attacked but maybe a bit less often.
