Is it a glitch? It happen three times in two attacks.
Twice I attack twice before my opponent hits back. One time it happens in the same second.
I can even attack three time.
Also not a join attack.

This happens due to Distraction debuff. Distraction effect can carry over from previous fight (which it did in this case). If a player receives another attack within 30 secs of receiving an aggressive action from another player, the system treats it as a group attack and they have a chance of missing a hit due to Distraction.
