it is my understanding that it is in fact impossible to do many things while you are under attack! taking any drugs is 1 of them (again to my understanding).
however I was in a active attack & got the message,,,
"(my target) was sent to hospital before FlaminRainbows was able to finish the fight"
I clicked to go to the targets page & it sais,,,
"ODed on xanax"

Is this a bug or can we take drugs while under attack?

Maybe...he was in hosp from attacker, then took Xanax which OD'ed...that's very possible since you can take drugs while in hosp.

I didn't send him to the hosp! so how did he get there?

Ahh, misread, thought another attacker sent him to hosp.

Taking drugs is not blocked when you are under attacker, neither is using boosters. Medical item usage is blocked.
