Dune bonus bug?

Is this a bug or does the Dune set only give the insurmountable bonus once per fight?

you know that insurmountable only works below 1/4 life, right?

I was below 1/4 health and got hit in the chest twice with the same weapon. First one triggered, second one didn’t

This may be due to Cauterize proc interfering with the Life calculation.
