Are you attacking / defending against a player with much higher dex than your speed?
That is a large part of hit rates
As Baro said it is very liekly you are attacking targets that have much higher dexterity than you have speed. If your opponents dex is 64x your speed, you simply will not be able to hit, ever. So train up!
However, you can get an accuracy boost by using more accurate weapons. The most accurate gun is the Benell M1 tactical. HOWEVER, it has a noticeably lower damage output. Both the Mag 7 and Aramlite have good accuracy (certainly better than your M16) with decent damage output. The m16 is not a great weapon, and an upgrade to a mag 7, arma, or enfield should be a priority.
You can also use things like tear gas to lower your opponents dex or melatonin to boost your own speed. Note that these are likely far too expensive for you right now. However, you can probably afford pepper spray, you will just have to be quick in using it as it expires very quickly.
Ther are a few other ways to boost your accuracy; you could use tracer bullets. You could try using the drug Speed for a temporary boost. But your best, most permanent option is going to be just training your speed.
