Why does it show the name of the person sometimes but other times not when I have been attacked

You were stealth attacked. Players who use stealthy weapons (you may see the stealth stat in any weapon, the higher the number, the higher the chance of a stealth attack to be successful) can attack you without being exposed. Read the wiki and read "Attacking" to learn more about stealth attacks.

You're likely just being hit because of the halloween event though so it's probably not worth the effort to find out who it is. Just ignore it until the event is over in a few days.
