Hello, the FAQ next to the captcha explains everything that we can make public about the tool. These checks will come up if the system detects suspicious, excessively repetitive or automated activity. The system will automatically remove you from the list once things go back to normal. Unfortunately, the nature of these checks aren't something that we can make public.
For the second item in your report - your uploaded image is broken/not loading, but I will assume you are referring to the captcha is off-screen in some manner. You can verify your phone and app zoom levels are set to 100% and if that doesn't resolve it, it is likely the same as this already confirmed report. Thanks!
