I think it's a random roll when you buy the weapon or armor or when it is spawned from a cache

How does it work?
Weapon damage and accuracy has range, quality is a representation of that range, with damage and accuracy being weighted equally.

Yes, each value is randomly generated upon the weapon being purchased.
Just to clarify, the quality will randomly generate but standard weapons are not possible to roll into yellow/orange/red quality like RW weapons. They can only be had via Caches or player to player sale. (Meaning generate a bonus)
Realistically quality doesn't really make a difference on on regular gear. There's so much accessible cheap Junk RW weapons that no one is really going to pay more for a regular weapon because of it's quality; outside of a few niche collectors maybe.
Shooby is back! Exception: sometimes you can change more for high quality Halloween weapons.
