This happened just half an hour ago. I was doing crimes, i am on crimes 1.0 still, and got through my usual just fine.
As my log shows, my last crime was an apparently successful swift store robbery. It didn't show me it was succesful, instead the screen got stuck, then gave me a 'you need to wait to have more energy in order to do this crime' when i hadn't clicked on anything. Then when i went back, i was in jail.
Someone busted me out before i even had a chance to get a screenshot, so i checked my logs. If i remember correctly, it should show i was either unsuccessful in committing the robbery, or tried another crime and failed, which would also be an error because as you can see, by that point, i had used up my nerve (my max nerve atm is 21)
I have a screenshot of the log here and my event page showing i was busted out of jail, but both don't coincide.

Friendly people can be a bit hard to find and you will most likely not find them in the forums, but feel free to reach out any time, i try my best to be a friendly and noobies are always fun to talk to.
Subjective, personal opinion of mine, and i can very well be wrong, but unless you are actively looking to make in game friends, which is great and a perfect part of enjoying the game for anyone that wants that; Torn is much more enjoyable when you take it easy, interact with the least people possible and stay out of mass game interactions (which is a big irony. Very big)
Less drama, less rude people, less chances at being harassed, bullied, etc.
All it takes is one interaction with someone in a bigger faction or with more big stat buddies than yourself for you to get mugged to hell and back, to make you never want to play again.
For some people the biggest sell of Torn is how easy and rewarded it is to be an asshole to others. The only way to reduce the chances of getting on the radar of that kind of people to near zero is by lurking and staying out of big open spaces, IMO.
Actual tip: dont play this game, log off, it gets worse by the day and most active players around are trolls and bullies, especially to noobs. You will probably find this out the hard way sooner rather than later.
If you still want to play, try to take it easy and find something you enjoy doing here. Read the guides but don't follow them to the letter if you plan to have any fun, stay away from big factions, forums and mass chats (Global, Trade, Hospital, Jail) interact as little as possible with active accounts unless you're looking for friends.
Find something you like doing, even if it's not what makes your stats and bank grow like all guides say.

I'm no expert, but i've failed/timed out plenty of Duke missions before so.
There's nothing you can really do about it. All missions have a limited time for you to complete them by performing the task and marking it completed, if you don't finish it in that time window, the mission is failed. Even if you finish the action afterwards, like by defeating the target with a hammer, you can't claim the mission as completed.
All you can do is keep doing missions as they come, all loosing that mission means is that you haven't completed it and you lost some agent standing (which as far as i know only affects your chances and proximity to getting higher difficulty missions with higher payouts). It will stay marked failed until you complete enough missions to 'push' it out of the list.
In the future you will probably get it again, so you will most likely get another chance at finishing and claiming it.
