No one can give you the answer you're looking for. It's just not been done before.
The only OD rate that was tested appears to be xanax: 2% according to Bogie's topic (thank you ;) )

As you can see, the info you're looking for isn't existing yet. Possibly because it's just very random and you need a very large pool size of people & drugs taken before one can really take any conclusion.

Every intake increases addiction, but OD's have an even larger rate of addiction.
Back when you used to be able to run a script and aggregate all play data, nerbas ran a query on overdose rates excluding non-drug using players and as I recall, it came out to 2.6%
Nope, it varies between each person.
There are only estimates for addiction values.
