Does the weapon XP bar indicate its ability building up to the stated accuracy and damage (ie: you're doing 58% of the Jackhammer's 71.31 damage/65.23 accuracy) or does it represent accuracy and damage above the stats indicated (ie: you're doing the Jackhammer's 71.31 damage/65.23 accuracy + 58%)?

It 'increases' said weapons stat, otherwise if someone had no XP for a certain weapon they'd have 0% of the base damage resulting in them not being able to hit.

The bonus should be over the weapon stats, logically. But the % on weapon exp does not mean the % value * weapon stat. The exact value on how much more damage/accuracy it adds, like many other things in TORN, is still a mystery.
However, the increase seems to be quite significant if you have 100% for a particular weapon.
The increase is said to be 25% in Damage and 25% in accuracy at 100% weapon experience. Not sure if that's absolutely true, but like 'Hospitalisation takes no CE', it seems to be one of the accepted truths of Torn. Though whether there's been any thorough test done in the weapon XP's case, like there has been for crimes, I couldn't say.
