You're going to probably want to use a unique name/ID for each of them rather than the human title but I'll leave that up to you.

Any idea why so many of each? Do they not have sub-names?
They represent different levels of addiction and sickness, just like the cooldowns.
For example, "icon57": "Minor Drug Addiction - Your statistics in attacking are slightly reduced (-4%)".
I don't know the exact ranges though.
