So every once in a while I'll attack someone I should be able to beat who just takes 0 damage from my attacks. Even shotgun blasts to the face do nothing.

The damage section just says attack mitigated.

Is this just a player who went all in on defense?

Pretty much. When your strength is 1/14th or less than their defense, you will do 0 dmg 100% of the time.

Just use the Strength boosting temp.
