I was holding a target, clicked "hosp" with 15 seconds left on the timer, but the attack "timed out" instead.

I had this same issue last week. I think the timer on the page and Torn's internal timer aren't synced. I clicked with 20+ seconds left and timed out.
By my logs I hadn't been attacked in > 1 hour, Sep attacked (but said he joined the fight)

Yeah, there were 1 attacker already in when I joined in, and 1 more person joined the attack right after. None of those shows up in the attack log either, its very strange..
I'll pass this to Chedburn to see if he can find anything wrong with the timer.
Were you on mobile? If you minimize the browser / app, or your phones screen goes off - I expect it's very likely the page timer can go out of sync, I have no idea how we'd resolve that however.
^this.. also it happens if you open other tabs while holding (on pc/laptop)
keep refreshing attacking page while holding... that will re-sync the timer
I wonder if it's possible for such timers to automatically refresh the time occasionally. I know this has also been a problem with the travel timers (and we added a refreshing system to that) I really don't know how else we'd keep them in sync under such circumstances.

No, this was on PC. I had a couple other tabs open (enemy walls) but those tabs were "passive". If I only use 1 tab every time I hit someone on a wall it would be very inconvenient.
The same thing happend on a chain as well, there were ~20 seconds left on the chain timer (was watching it on other tab) and when then I clicked hosp but somehow the chain had timed out.
Asked around in my faction about the issue and was told the timers have been out of synch for weeks.
I have done some testing, it seems the timers goes down ~20% slower when viewing another tab.
Opening a new window will keep the timer accurate, simple workaround for the users is to pull the tab out when they plan on attacking.
Not sure if an actual fix could be using cache memory or smth and use the local time on the device?
