Minus / plus is there for a reason :)
Effective stats is used in both attacking and defending.
If you are using doctorn your effective stats are the stats you actually use in battle. These include drug and other bonuses.
Your normal stats are just the same without bonuses.

I dont use drugs but my effective stats are +% due to education and merits. and yes im saying it because ive seen it on my DocTorn.
I dont have minuses :p

Yes, the percentage is how much on top of your normal stat is added on in combat.
To keep it simple, say you have 1,000 of a stat. You get 50% bonus on top of that, you still see 1,000 of the stat, but when attacked, or attacking, the stat becomes 1,500 due to the 50% bonus during the fighting, but is not added to your regular stat. This is because how much of a stat you gain while training is tied to how much of the stat you already have as well as other reasons but they aren't as important to us players to know, nor do I imagine I know them all.
