Seems like the best way indeed. I don't think Torn would be "happy" with every user being in a separate database for such purposes as you provide :P So don't think they will aid it in any way.

As far as I can tell I stay within the guidelines provided, and people seem to like the site.

I just completed this very task.
Do the following.
Get the API call to torn -> city stats and look at the total player count.
start at ID 1 and start grabbing user -> profiles. Be sure to save error code 6 along the way. Once you hit the same number of real players as reported in city stats then you’re at the end and can delete all error code 6 profiles. From there you have a highest known player ID and can just check the next number until it gets created.

This process took me a little under two months while running at about 40 calls/s.

Does "users total" take into count test accounts, dummy accounts, NPCs, etc.?

If those accounts are not counted towards "users total" then you are probably behind a bit. Not that significantly tho.
