Does *bang* trigger has been setted on first play, or changed everytime when playing the game? i don't know why every player took so long time to hit the button. or might be they just not yet be prepared for losses?

Each time you click to shoot there is a chance of a bullet. It is not pre-determined at the start which chamber contains the bullet.
People wait before shooting because they are multi-tasking and not paying attention or setting up a mug/self hosp usually. Or it could be connection issues.

The self-hosp is set up before they pull the trigger. This method is safer than ghost trade (unless you forgot to turn off revives!).
