I don't see anything regarding it here although I'm sure it's probably there, but can someone explain what happens during a drug cooldown, booster cooldown from chocolate, and medical cooldown from first aid/morphine? What does this stuff affect? Are there certain things I can't or shouldn't do during cooldowns? Are there things I need to do that are beneficial during this time? I don't understand it very well. Thank you in advance!

It just means you can't use anymore of tha particular item. Say you have 24hr booster CD and you eat candies to max it then train. You will not be able to eat more candy take beer cans fhc things like that until the timer is under the 24 hrs mark again
During Drug cooldowns, you may have buffs/debuffs from the drugs.
For eg, xanax reduces your effective battle stats. Vicodin buffs you.
Regarding other types of cooldowns, you can't use that item anymore if max CD is reached. No penalties.

to add, without perks booster cd works like this and is capped at 24h. Med cd works like this and is capped at 6h. For these there's no secondary effects tied to the respective cds (secondary effect from rad poisoning is it's own cooldown)
Drug cd works a little different, you can't stack it up. You can't take any drugs while under drug cd, irregardless of it's current time accrued. Additionally, every drug has different passive effects that will effect you while your drug cooldown is running, found here under the "Effects & Cooldown" tab of the chart. Each passive effect after taking a certain drug will effect you until your cd hits 0, then it will go away.
Ofc there's other side effects due to addiction with drugs, but OP asked about cds only; addiction is separate from the drug cd itself
