A patient undergoing brain surgery is kept awake because the surgeons want to know if they hit some important part of the brain while operating. So they ask patients to play an instrument or talk to them while operating. So if they end up hitting an important part of the brain that controls either of these things, the patient will immediately reflect it in their activities so the surgeons know to stay away from that region of the brain.
Also this is only possible because brain does not have any pain receptors.
