A friend added me, it’s says under friends 1 person has added you. How do I add them back?

If you know who it is (or suspect you know) you can add them by username on your Friends List page or by using the Add Friend icon on their profile page.
If you don't know who it is, you could pay someone in a Detective Agency to do a Friend & Foe reveal, but it costs around 24m usually and not really worth it unless you have loads of money and are wildly curious.
Some people use friends list to track mugging targets. It is not like in other games where you become mutual friends when adding a friend.
