So I've had the egotist title since the day titles were released but I've also done a ton of other things since then and my title has never changed. My factions gone through chains wars, I've bought stocks, I've used 500+ xanax, and yet I'm still an Egotist?

Your title is decided by which attribute you are ranked highest in among all other torn players.You are an Egotist since you are ranked higher in bounties placed than any other attribute.
For your title to change,you'll need to increase the rank of one of your attributes higher than your rank in Bounties placed.
For example,If you are ranked 1000 in bounties placed and ranked 1005 in drugs used then lets say a week later you have used enough drugs to move to rank 999 in drugs and you are still are at rank 1000 in bounties placed then your title will soon change from Egotist to Druggy.

Looking at your personal stats you dont seem to rank high in many things
There is no title that deals with stocks btw

Yeah I wish that was the case too, its in the system already so why not just show them.
Because that would make it too easy? If you are determined you can probably get a pretty good idea by checking the personal stats of other players who hold a title that interests you.
