Does OD require rehab or do people go to rehab to fix the zero'd happy only?
I was of the understanding it creates a debuff that needs rehab to fix - whats the truth of it?

The debuff is a longer than usual drug cooldown, rehab doesn't help with that. ODs also give more addiction than others, so that's another reason to rehab if you care about addiction effects.
Crimes and gym training are less effective hen your happy is low. So you want to get it back up. When I OD'd earlier this week I was in the middle of a race and CBA with rehabbing at the time so I just took a yellow easter egg to get back up to full happy instead.
You only need to rehab one unit to reset your happiness back to full. Natural degradation over time will normally take care of any residual addiction. You will still have an extended drug cooldown (23 - 24 hours or so) that will give you an extended period of drug free time for natural reduction to happen.
If you are addicted to the point when the brain icon is showing, your battle stats will be debuffed. The colour of the icon will change as you become more addicted and that will indicate how much of a debuff in battle stats you will have.
Over-dosing does not affect drug addiction level any more than normal drug use does.
Also, drug addiction levels or having rehabbed doesn't affect your chances of overdosing. That's all RNG.
OD absolutely does affect addiction, per Wiki. Except for Cannabis and opium. Drugs wiki

Rehab doesn't reduce the risk of an OD. I have seen it too many times where faction mates or myself have just rehabbed and then OD'd on the next Xanax.

You are right but for most drugs, an OD will zero out your happy and happy is very slow to refill naturally. To quote the Wiki "Happy or happiness plays a role in all aspects of the game, although the precise extent of its influence in most areas remains uncertain."
So being very low in happy is not a good idea in general. As I said, rehab one unit and happy will be refilled.
I never said it reduces chance of od but when you od you gain extra addiction points. the extra addiction points over time will increase ur rehab fees
