What are the overdose chances for Xanax and other drugs, and why do overdoses seem so common?

No source gives a per-use overdose percentage for Xanax or for any drug except Cannabis, which was 1 in 2500 in a 2022 wiki capture^[1]. The wiki says each drug has its own small chance^[2]. Players from 2014 to 2020 reported personal rates of about 4% to 18%, which they found high^[3][4][5].

What the wiki documents

• Any time a drug is taken there is a small chance to overdose, except Opium and Love Juice, which cannot be overdosed on^[2].
• Each drug has a different chance: Cannabis is extremely low and Ecstasy relatively high^[2].
• The 2022 capture gives 1 in 2500 for Cannabis, and 1 in 500 during the Cannabis Day event^[1].
• No source gives a Xanax figure. A 2019 asker could not find a guide with per-drug chances^[6].

What affects the chance

• Addiction, frequency of drug-taking, number taken and time since the last overdose have no impact on the chance^[2].
• A forum post from 2023 says each drug always gives the same effects and the same addiction points^[7].
• The 2022 wiki capture lists a faction 'Overdosing' upgrade (Toleration branch), 3% less chance per upgrade, to a maximum of 30%. The 2025 page does not mention it, so its current status is unconfirmed^[1][2].
• A 7-star Nightclub passive special gives a 50% drug overdose chance reduction. It is listed in wiki captures from 2023, 2024 and 2025^[8][9][10].

What an overdose costs

• Xanax: removes 100% of Energy, Nerve and Happy, adds 5000 minutes of hospital time, and raises the cooldown to 24-25 hours with more addiction points^[11].
• Opium cannot be overdosed on^[12].
• Effects for the other drugs are not in the current sources.

Why overdoses seem common

Reports are dated, from 2014 to 2020, and predate the 2025 wiki, so they may not reflect current rates. Examples: 4 of 22 drugs (18.2%) in 2018^[3], 3 of 11 tablets in 2018^[13], 3 of about 8 Xanax in 2019^[14], and 3 of 17 LSD in 2019^[15]. One player in 2014 reported 8.2% and said most players he knew were at less than half that^[4]. In 2020 a player reported 90 overdoses with 1,126 LSD taken, listed as 7.15%^[5].

Small samples explain the short reports, since a few overdoses out of a handful of drugs swing the percentage widely. They do not explain the 1,126-LSD and 8.2% reports. Those conflict with the wiki's 'small chance', and 90 of 1,126 works out to about 8%, not the 7.15% shown^[5][2]. TornLife found no staff explanation of the gap.

Xanax overdoses also stand out because each costs 5000 minutes of hospital time and empties Energy^[11]. That is the writer's reading, not a sourced finding.

Where sources disagree

• The wiki says the overdose chance is small^[2]; players reported rates of 7.15% over 1,126 LSD and 8.2% overall, which are not small samples^[5][4].
• The wiki says frequency and addiction do not affect the chance^[2], yet players posted reports of frequent overdoses despite faction perks^[16][13].
